What is Melasma?

Melasma is a skin disorder characterised by patches or spots of skin with a darker shade than your actual skin tone. You may sometimes hear it called the “mask of pregnancy”, as it commonly happens during pregnancy. Melasma generally goes away on its own without requiring any treatment, but in many cases, it will require dermatological treatment to resolve.

What Causes Melasma?

Our skin colour is a result of melanocytes, cells that store and produce the pigment melanin, which provides pigmentation to our skin, eyes, and hair, absorbs harmful ultraviolet (UV) radiation from the Sun, and even protects other cells from damage due to long-term sun exposure. The amount of melanin produced by the melanocytes differs from person to person, resulting in differences in skin tone. The more melanin you have, the darker your skin tone is. Interestingly, freckles are also caused by clusters of melanocytes in your skin.

The exact cause of melasma is still unknown, but what is known is that it results in an overproduction of melanin. It is suggested that melasma develops after an as-yet-unknown trigger causes the melanocytes to go into “overdrive”. Another theory is that it is due to a “photoageing disorder in genetically predisposed individuals”.

Whatever the actual cause, melasma can be triggered in various ways. The two more common triggers are exposure to sunlight and hormones.

  1. Sunlight

    Heat, UV, and visible light are known to stimulate melanocytes to produce more melanin, which causes melasma to occur gradually. These areas typically receive the most sunlight, which may also be the leading reason why melasma affects the face, neck, and arms.

     

  2. Hormones

    Some studies suggest increased estrogen, progesterone, and certain melanocyte-stimulating hormone levels can cause melasma. Birth control pills and hormone replacement therapy may also be influencing factors. For pregnant women, melasma may disappear on its own sometime after giving birth.

     

  3. Thyroid Disease

    Located in your neck, the thyroid gland produces hormones that assist with various essential tasks all over the body, from regulating your heartbeats to breaking down the food you consume. Thyroid dysfunction can potentially cause melasma since it is related to hormones your body needs, but further studies are needed to find a connection between the thyroid and melasma.

     

  4. Family History

    If you have one or more blood relative(s) who previously had melasma, you may be at a higher risk of melasma.

     

General Symptoms of Melasma

Melasma can occur anywhere on your skin but commonly affects your face, jawline, neck, shoulders, and arms. They may appear tan, brown, greyish brown, or bluish in tone; some also report seeing freckle-like spots appear. In some cases, these patches may also appear reddish or inflamed. Also, melasma does not cause other physical symptoms, such as itching.

Melasma Treatment Aftercare

Melasma can occur anywhere on your skin but commonly affects your face, jawline, neck, shoulders, and arms. They may appear tan, brown, greyish brown, or bluish in tone; some also report seeing freckle-like spots appear. In some cases, these patches may also appear reddish or inflamed. Also, melasma does not cause other physical symptoms, such as itching.

  1. If your melasma is triggered by sunlight exposure, use a broad-spectrum sunscreen with at least SPF 30 daily when you go outside.

     

  2. Adopt sun-smart behaviour, such as wearing wide-brimmed hats, sunglasses, and comfortable, loose-fitting, long-sleeved clothing or skirts/pants to minimise sun damage.

     

  3. You may be given a topical cream to apply to your skin at regular intervals to help keep it in good shape. If you exhibit any allergic reactions when using the cream, stop using it and inform your practitioner immediately.

     

  4. Regular follow-ups with your practitioner help to monitor your skin’s condition and plan future treatment sessions when necessary.
